If an Emergency Contact Form only asks for one emergency phone contact, but allows users to add more phone contacts, the form will print as blank.

If the Emergency Contact Form asks for two emergency phone contacts, and allows users to add more phone contacts, the form will print correctly with all information.

Navigate to onRecord > Settings > School Forms.
Click Edit Form next to the appropriate emergency contact form.
Click on the tab that contains the User Emergency Phone Contact block.
Click on Block Settings.
Select 2 from the Number to Show dropdown.
Click Save.
Click < Back to School Forms in the upper left corner.

Note: If a user has already viewed the emergency contact form, that form can no longer be edited, but you can create a new editable copy of the form.
