Why is there a page blank when trying to create an existing constituent as an Admin?

When attempting to use the "Create as Admin" feature on a constituent record, I am seeing a blank page with only "Next" and "Cancel" buttons. Screenshot of described behavior:

This can occur when the constituent record does not have an email address. In order to resolve this, simply add an email address to the respective constituent record, and then click on the "Create As Admin" link again.
