Who removed my Sphere User

An organization would like to know why a username was removed from their Sphere account. There can be a number of reasons why a user was removed but here is a list of possible reasons. 
  1. Someone from the organization, with Administrative access, has removed or deleted the user from the account.
  2. The Sphere User has attempted to change their password too many times in a short period.
