Error: ERR_STUDENTMEDICALALLERGY_DATESOVERLAP

Possible Cause 1: The user has selected the same Allergy twice. For example, the Parent has selected "Other" for two separate Allergy entries. Medical Forms will not allow a user to select an Allergy option more than once.

To Resolve: Remove one of the entries. If a student has more than one allergy that requires using the "Other" option, the school may consider adding an "Other 2" option or expanding their Allergy table.


Possible Cause 2: The Allergy information in the form conflicts with the Allergy information that exists on the Student's Profile because A. the medical form was assigned to the Parent and then an Allergy was manually added to the Student's profile before the form was submitted or B. the parent moved an existing Allergy to a different position within the form.

To Resolve:
  • Remove the manually added Allergy from the Student's Medical Profile
  • OR Remove the Allergy from the form
  • OR Remove the Student and their recipients from the Medical Form, save, then add the Student and their recipients back to the form to create a new copy of the form for them to complete. Note: this will refresh the form, so any information the user entered will be removed.


Error: ERR_STUDENTMEDICALCONDITION_DATESOVERLAP

Possible Cause 1: The user has selected the same Condition more than once in the form.

To Resolve: Remove one of the entries. The School may need to expand their Condition options or provide an explanation in the Condition notes if there are multiple instances of a single condition.


Possible Cause 2: The Condition information in the form is conflicting with the Condition information that exists on the Student's profile because A. the form was assigned to the Parent and then a Condition was manually added to the Student's profile before the form was submitted or B. he parent moved an existing Condition to a different position within the form.

To resolve:
  • Remove the manually added Condition from the Student's Medical Profile
  • OR Remove the manually added Condition from the form
  • OR Remove the Student and their recipients from the Medical Form, save, then add the Student and their recipients back to the form to create a fresh form for the user to complete.  Note: this will refresh the form, so any information the user entered will be removed.
