The top of an Embedded DIY Form does not appear correctly on my website

When taking the HTML embed code and inserting it into a website, the top of the form appears differently than the preview of the form. The may text appear on a different line and some images appear out of place. 
This is an issue that can appear if the area in which you embed your form is not large enough to accommodate the size of your form and the images and text in the top portion of it. In order to resolve the issue, you need to make sure that you use the Embeddable template for the form and not another template style. If you are not using that template with your DIY Form, you can switch that template using the steps below within eTapestry:
  1. Click Management
  2. Click DIY Forms
  3. Click Edit for the form in question
  4. Click the Edit Style button
  5. Click the Swap Template button
  6. Select the Embeddable template
  7. Click the Swap to this Template button
  8. Click Save
  9. Click Go Live to publish the changes
