If you are receiving this error, this means a field exists that is missing codes. Since you are likely already working within the Forms Manager, please follow these steps below from your Forms Manager page to resolve:

1. Open the form that threw the error message by double-clicking it
2. Once the form is open, search for the field specified in the error message
3. Once the field is located, highlight the field > Along the right side of the page, there should be a link to click called 'Edit Codes' > Click this link
4. Once 'Edit Codes' is clicked, you are brought to a Coding Table window > To add codes to the field, check the boxes for the codes in the Available Codes column > Click the right-facing arrow to add them to the Selected Codes column > OK
5. Save & Close

You should now be able to activate your form.
