Default country is not appearing in Web forms

When purchasing online there is no pre-populated country in the personal information tab.  
The name of the country must be listed as United States. If the country is named anything else, or there are duplicates, this can cause issues with your default country. To confirm the country name: 
  1. Click  Administration > Click Countries and states
  2. Click the arrow next to the United States option
  3. Make sure Mark as default is grayed out. If not, click Mark as default
  4. Click Edit
  5. Confirm Description is exactly set to United States 

Steps to Duplicate

  1. Click web > Manage donation forms (happens on all web forms)
  2. Click the arrow next to a designation > Click on the URL
  3. Select an amount and click Continue 
  4. Notice that no country pre-populates the Country box in the personal info tab
