When using Request or Organization Primary Contact fields, sometimes these fields will not remain Required despite being set to be. Please follow the steps below to resolve this issue.
1. Launch Forms Manager 2. Deactivate and open the Inactive Form 3. Click on the drop down menu for the field categories 4. Select Request/Organization Primary Contact 5. At the bottom, check the box that reads "Organization Primary Contact is required" or "Request Primary Contact is required"
