Error: Cannot insert duplicate key row in object 'dbo.CONSTITUENTSITE' with unique index 'UIX_CONSTITUENTSITE_CONSTITUENTID_SITEID'. The duplicate key value is (xxxxxxxxxx) - when creating new organizational records

Users have found that when a System Role contains the Add Form feature permission "Individual, Spouse, Business Add Form". They are unable to create new organizational records from the "Add Individual" task under the Constituent functional area.

Users receive the follow error when they attempt to save:
Data could not be saved. Cannot insert duplicate key row in object 'dbo.CONSTITUENTSITE' with unique index 'UIX_CONSTITUENTSITE_CONSTITUENTID_SITEID'. The duplicate key value is (xxxxxxxxxxxx).
To work around the error, users can create new organization records using the "Add an Organization" task under the Constituent functional area.

We're currently evaluating this issue for a fix in a future release.
