From Field blank and grayed out on Opt Out Email tab when submitting data to EmailFinder

When submitting data for an EmailFinder screening, the From field may appear blank and grayed out, and because it is a required field, the error, "Required field missing" will appear when clicking Submit Data.

Download and install the latest version and patch, if applicable. 

Alternative Solution: 
Log into Raiser's Edge with a user that has not used the Data Health Center before and create the export.
If necessary create a new temporary user (BB138504) for the export (it can be deleted after creating the export)
