1.  Have the primary contact at your organization complete and sign the Hosting Change Authorization Form, being sure to note the date and time that you wish your database to be restored to.
2.  Click 'Create a Case' in the Additional Assistance box on the right and reference this solution ID. 
3.  Fax the authorization form to 843.216.6100, noting your case number, and a support analyst will be in touch with you once the necessary changes have been made.

*Note: any data entered or change in the database after the restore time will no longer be present in the database.
