Follow the below steps until the issue is resolved:

  1. Open affected Action record.
  2. Click the Notes tab.
  3. Verify that this action has the missing notes (or any) listed here.
  4. If notes are logged for this Action record, verify that there is text entered in the box below the "Description" field.
  5. If there is no text in this field, enter the desired notes text in the Action text section.
  6. Click Save And Close.
  7. Run the Action Detail Report again.
