Error: A Page Error occurred\r\nURL when adding a document to a shared assignment

When adding a shared assignment from the Edit assignments page or from a gradebook, you have the option to include a document.  If the document you select has an apostrophe in the file name, you will receive the following error:

Download and install the latest patch, which contains all fixes from previous patches. If you are running an older version, download and install the latest version and then the patch. 

Alternative Solution:

Remove the apostrophe from the file name
Add the document to the assignment from the Add shared documents screen after the assignment has been created.
