This occurs when fields in your import file are not mapped in your import process. To resolve this, follow these steps:
  1. Go to Administration>Import
  2. Highlight your import process and click Edit
  3. Verify that all necessary Batch Template Columns are mapped to the corresponding Import File Column
  4. If your import contains any Collection Fields (those that may have multiple values like phones, email addresses, etc) highlight the row and click Map Collection Fields to map those columns as well
  5. Save the import process and run it again
