Query categories are missing from the Category drop-down menu on the main Query page

In the Query functional area, users may filter the queries that they see based on Category.  However, in some instances the query categories are not available in this drop-down list, even though they are present and enabled in the Query Category code table in Administration.  The query categories in question also appear in the Category drop-down menu on the Save Query screen, and are associated with several existing queries.

Download and install the latest version and patch, if applicable.
