In order for a constituent record to be updated or created via a survey, the survey must have a question of the type "Constituent Registration Information" (CRI) and it must include the email address.  Without that, the survey will not update or create any constituent records. Once a question of that question type has been created requesting the email address, other questions of varying question types will work to populate constituent records.

Additionally, if the CRI question is not properly configured in step d, "Select Layout and Processing Options", for the option "Response Processing if Duplicate Email Address is Detected" to either "Accept the response and update the Profile of the constituent if improvement" or "Accept the response and always update Profile of the constituent", existing constituents will not be updated by the survey.
