The first step is to create a defined field called Tribute Name:
  1. Click Management
  2. Click User Defined Fields
  3. Click on the category where you want to store the field (i.e. Base)
  4. Click New Defined Field
  5. Name the field Tribute Name
  6. Select Text as the Data Type
  7. Click Next
  8. Select the box for Transaction as the field application (do not check off any other boxes)
  9. Click Next
  10. If you would like to utilize either of these options then check the appropriate box (do not check the box to make the field required)
  11. Click Next
  12. Select Text Box as the Display Type
  13. Click Save and Finish

The next step is to create a defined field called Tribute Type. This will track whether the gift is being made in honor of or in memory of an individual:
  1. Click Management
  2. Click User Defined Fields
  3. Click on the category where you want to store the field (i.e. Base)
  4. Click New Defined Field
  5. Name the field Tribute Type
  6. Select Text as the Data Type
  7. Click Next
  8. Select Transaction as the Field Application (do not check off any other boxes)
  9. If you would like to utilize either of these options then check the appropriate box (do not check the box to make the field required)
  10. Click Next
  11. Select Selection from a set of values as the Display Type
  12. Click Next
  13. In the Add value tile, type In Honor Of
  14. Click Add Value
  15. In the Add value tile, type In Memory Of
  16. Click Add Value
  17. Click Save and Finish

You will now link these defined fields to your DIY page:
  1. Click Management
  2. Click DIY Forms
  3. Click the Edit to the right of the DIY page you want to add Tribute information to
  4. Click Add Item
  5. Click Add Fields
  6. Click the drop down box and select the category where you stored your tribute fields (i.e. Base)
  7. Click the Tribute Name and Tribute Type options
  8. Click the + sign to the left of Tribute Name
  9. Enter the desired label for this field that you'd like for donors to see on the page
  10. Click the + sign to the left of Tribute Type
  11. Enter the desired labels for this field and values that you'd like for donors to see on the page
  12. Click Update
  13. Click Go Live
  14. Click Yes, Go Live
  15. Click Replace
When a donor submits a transaction via the DIY form and enters tribute information, the Tribute Name and Tribute Type selected by the donor will populate on the Transaction screen under User Defined Fields.
