Error: Error Code DTS_E_OLEDBERROR. An OLE DB error has occurred - when refreshing ETL

When refreshing, or resetting and refreshing, the ETL, it fails with the following info:

Download and install the latest patch, which contains all fixes from previous patches. If you are running an older version, download and install the latest version and then the patch.
