UK Version - Paperless Direct Debit Instruction fields are missing from the Recurring Gift Payment Details Edit Form or Pledge Payment Details Edit Form when specific bank selected

When using paperless direct debit payment method, the direct debit instruction fields on the Recurring Gift Payment Details Edit Form or Pledge Payment Details Edit Form are removed when specific bank is selected on the Account dropdown.
Edit the offending Constituent Financial Institution to add address data via Constituents -> Constituent Financial Institution ->select bank in question -> edit.
