Error: Unable to execute action for class AdHocQuerySmartQueryOpenAction. The select field is not contained in the output definition. when editing a user defined smart query with a selection.

After upgrading from patch 119 to patch 125 users are seeing the above error when attempting to run or edit a user defined smart query.  Creating a new user defined smart query results in the same error after the first run regardless of the selection being referenced in the query.
Download and install the latest patch, which contains all fixes from previous patches. If you are running an older version, download and install the latest version and then the patch.
