Typically, this occurs because the email message was sent while logged into a URL that is not the Primary Site URL

If you're are logged into the Primary Site URL and are using Outlook 2013, please see the information below.

By default, Microsoft Outlook 2013 is configured to block pictures that are links within a message that download pictures from an internet location. Because of local security settings these pictures will not be available to download on the the email message in the header

Please contact your email exchange server administrator to update the security and trust settings for Outlook 2013. 

This can be for several reasons including:
  1. Corrupt outlook/windows profile
  2. Trust settings in outlook 2013
  3. Temp folder and registry key errors
