1. Navigate to Communities > Forms > Contact
2. Click New
3. Name the form
4. Select a Fund
5. Click Submit
6. Hover over the form and click Website Info
7. Select Form Layout, and click choose a type of layout
8. Click Save
9. Click Contact Form Option
  • We do recommend the enabling of CAPTCHA. CAPTCHA help to distinguish whether the form's user is a human or a computer. CAPTCHAs are used by many websites to prevent abuse from bots, or automated programs usually written to generate spam.
  • How to turn it on? This can be done by marking the Enable CAPTCHA option. 
10. In the Confirmation Text section, add email address
11. Click Finish
12. Select Form Customization
13. Click Select beside the Contact Us form
8. Activate the fields you want to appear on your form
9. Click Save & Done 
11. Navigate back to the Webinfo Checklist and click Preview
12. Click Publish This Event Now
  • Note: To make the form live, choose Activate and click Publish This Event Now
