How can I add a middle name, or middle initial, on a donation form?

The Canadian Revenue Agency requires a full name, including middle initial, on official donation receipts. How can I add middle name or middle initial as a required field to the donation form?

This is resolved in the latest version of NetCommunity. If this behavior is persistent, please confirm you’re on the latest version of NetCommunity.



In NetCommunity version 6.64 patch 12, and later: 
  1. Edit the donation form part
  2. Mark the checkbox to Include the Middle Name field in the Additional Fields section
  3. Mark the checkbox to Require the Middle Name field in the Additional Fields section (if desired)
  4. Click Save


Alternative Solution to upgrading:
  1. Edit the donation form part
  2. Click the Language tab
  3. Choose Billing Information from the drop down menu for the Category field
  4. Change the label for first name to something like: First and Middle name or First name and Middle Initial
  5. Click Save

Steps to Duplicate

Steps to duplicate:
  1. Go to a donation form page
  2. See there is no field for middle initial/middle name. The eReceipt will use the billing information fields from the donation form
  3. Edit the donation form
  4. See there is not a way to include an additional field for middle name/initial
