1. Navigate to Communities>Forms>Contact
  2. Hover over your form and click Reports
  3. From the Run quick report dropdown, select People: All Initiative Contacts
  4. Click Run Report
  5. From Group 1 dropdown, select Form Entry Date
  6. Select the appropriate syntax (=, >, <, etc)
  7. Enter the Date and Time in the following form: 1/1/2015 9:15 AM (an example)
  8. Click Next
  9. Add the desired fields, Form Entry Date under Contact Info may be helpful
  10. Click Finish
  11. Click No- Run report immediately
  12. Click Run Report
