This typically occurs when the relationships to the applicant are not Matched before processing.

To prevent duplicates from being created search for relationship matches before processing the form:
  1. Navigate to Education Edge>NetCommunity Plugin
  2. Click the link to process submitted forms
  3. Select the applicant in question
  4. Click View Relationship Matches
  5. For each relationship listed, click Add a Match
  6. Search for an existing match
  7. If a match is found, double click the record to mach it
  8. Repeat, if needed
  9. When finished, click OK, then Process Form
