Payment page is requesting "Valid From" and "Issue" fields

In Chrome, when users are entering credit card information on the Advance Donation Page, the fields "Issue Number" and "Valid From" appear before the user enters the credit card information. Once the user begins to fill out the required fields, the "Valid Form" or "Issue" fields disappear.  
Clear your browser's cache and cookies.

Steps to Duplicate

  1. Open Chrome Browser 
  2. Navigate to a donation page
  3. Below Expiration notice both "Valid From:" and "Issue Number" fields appear 
  4. Enter card information
  5. Notice both "Valid From:" and "Issue Number" fields are removed
